TMJ, Bruxism and Orofacial Pain in Alphaville
Jaw pain, clicking or popping sounds, difficulty chewing or opening the mouth, and muscle tension warrant investigation.
At Donegá Odontologia, we evaluate TMD, bruxism, and orofacial pain on an individual basis, seeking to understand the origin of the symptoms before defining the treatment.

What are the main symptoms of TMD (Temporomandibular Joint Disorder)?
Temporomandibular disorders can manifest in different ways, and not all patients experience the same symptoms.
Some signs that warrant evaluation include:
Jaw pain or pain in the TMJ area.
Pain when chewing
Difficulty or limitation in opening the mouth.
Clicking or other joint noises
Feeling tired or tense in the facial muscles
Pain in the temple area
Episodes of jaw locking
Headaches that need to be evaluated in conjunction with other clinical signs.

What do we assess and treat in this area?
Muscular TMD
TMJ joint
Orofacial pain
Pain and tension in the masticatory muscles
Bruxism and clenching with clinical repercussions.
Limited mouth opening
Clicking and functional changes in the TMJ (temporomandibular joint)
Pain associated with chewing.

TMJ pain: when to seek evaluation?
The TMJ is the temporomandibular joint, located in front of the ear and responsible for jaw movements.
Pain in this area, stiffness, limited movement, or difficulty chewing are signs that warrant evaluation.
Diagnosis depends not only on a single symptom or isolated test. Medical history, clinical examination, and, when indicated, imaging tests are all part of the investigation.

How is TMD and Orofacial Pain assessed?
The assessment begins with a history of symptoms, factors that worsen or alleviate pain, habits, sleep quality, and impact on daily routine.
During the clinical examination, we assess mandibular movements, masticatory muscles, the temporomandibular joint, and pain characteristics.
Imaging tests, such as magnetic resonance imaging (MRI) or computed tomography (CT) scans, are only requested when there is a clinical indication.

How does TMD treatment work?
Treatment depends on the diagnosis and type of TMD presented by the patient.
Whenever possible, we prioritize conservative and individualized approaches.
Depending on the case, the plan may include guidance and self-care, exercises, manual therapy, occlusal splints, electrotherapy resources, laser therapy, and monitoring of progress.
Not every patient needs a splint, and an occlusal splint alone does not resolve all cases of TMD (temporomandibular joint disorder).

Does a bruxism splint resolve TMJ disorders?
An occlusal splint may be indicated in certain cases, but it is not a universal treatment for all jaw pain or all TMD (temporomandibular disorders).
The indication depends on the diagnosis, the pattern of symptoms, the joint condition, and the treatment goals.
Therefore, signs must be planned and monitored individually.

What are the goals of the treatment?
Reduce pain and discomfort
To restore or improve mandibular function.
Reduce limitations in chewing or opening the mouth.
To educate the patient about factors that influence the symptoms.
Control overload when clinically relevant.
To improve the ability to cope with recurrent episodes.

Are bruxism, snoring, and sleep the same thing?
No. Sleep bruxism, snoring, and obstructive sleep apnea are different conditions and require specific evaluations.
In Sleep Dentistry, the dentist can participate in the management of snoring and obstructive sleep apnea in selected patients, including the use of intraoral devices when indicated.
A diagnosis of sleep apnea requires medical evaluation and appropriate tests, such as polysomnography.
Frequently asked questions about TMD and Bruxism
When is an MRI of the TMJ necessary?
Magnetic resonance imaging (MRI) may be indicated in specific situations, especially when evaluating the internal structures of the joint can modify the diagnosis or treatment.
Can TMJ cause headaches?
Some types of headaches may coexist with or be related to the function of the masticatory muscles, but headaches have diverse causes and need to be correctly differentiated.
